Only puppies, dogs with medical reasons, or dogs with special needs will be fed during daycare hours. Each dog owner is responsible for providing food for their dog if it must be fed for those reasons. The food may be provided on a daily basis or a larger supply may be left at the daycare.

I understand and agree that The Velvet Snout, LLC is relying on my representation of my cat’s health and behavior including but not limited to showing aggression or threatening behavior toward any other person or cat, that my cat(s) is/are in good health and have not harmed or shown aggression or threatening behavior towards any person or any other cat. I further understand and agree that The Velvet Snout, LLC and their staff and volunteers will not be liable for any problems that develop.

I understand that I am solely responsible for any harm caused by my cat(s), including to any other pets, to employees, the equipment, facilities, or other property while my cat(s) is/are attending The Velvet Snout, LLC.

I understand and agree that attendance in overnight lodging is not without risk to my pet. The Velvet Snout, LLC and its staff agree to exercise all due and reasonable care to prevent injury, illness, or loss of my pet. However, in the event of illness, injury or loss, the owner and employees of The Velvet Snout, LLC shall not be held personally liable for such injury, illness, or loss.

I hereby grant permission to The Velvet Snout, LLC to act on my behalf and in my pet’s best interest, by obtaining veterinary care at my expense if deemed necessary for illness or injury. I further agree to assume full financial responsibility for any and all expenses involved in the event of an emergency. The Velvet Snout, LLC will make every effort to contact me and/or my designated emergency contact; however, if time does not permit or if The Velvet Snout, LLC is unable to reach me or my designated emergency contact and veterinary care is urgently needed I give The Velvet Snout, LLC permission to seek the care needed at it’s own discretion.

I understand and agree that if I fail to provide proof of current vaccinations or my cat’s vaccinations are found to be expired or otherwise incomplete, The Velvet Snout, LLC has the right to refuse service until current proof is provided. Furthermore I certify my pet is in good health and has not been exposed to rabies, FVRCP, or any other communicable diseases within the last 30 days.

I agree that my pet may be photographed or videotaped during any daycare visit, boarding stay, grooming, and/or training session, and these may be used for marketing or staff training purposes.

I understand my pet will only be released to me or to an agent I authorize in writing. I understand and agree that all charges become due on the date the pet(s) is to be picked up. The scheduled pet pick up date may be extended with staff approval. The Velvet Snout, LLC shall have and is hereby granted, a lien on the pet(s) for any and all unpaid charges resulting from caring for pet(s) at the facility. I further agree in the event charges are not paid within ten (10) days after charges become due in accordance with this contract that any pet abandoned or not paid for (10 days past scheduled pick up) can or will be found a new home at the expense of the pet owner. I specifically waive all statutory and legal rights to the contrary. I understand that abandonment of my pet(s) is punishable under Texas State Law by fine and can be punished under the Texas Animal Cruelty Act as a Class A Misdemeanor under Texas Penal Code 42.09 which carries a $4,000 fine and up to a year in jail. The Velvet Snout, LLC will report pet abandonment to the authorities.

It is the owner’s responsibility to inform The Velvet Snout, LLC of any existing health conditions or any new health conditions as they are identified. On admission, all cats must be free from any conditions which could potentially jeopardize other cats. Cats who have been ill with a communicable disease in the last 30 days will require veterinary certification of health to be admitted or re-admitted.
